Vision Insurance Plans
We accept these Vision Plans:

VSP
We accept VSP which is a large vision care plan. Each plan differs as to the amount of coverage. When scheduling with our office please have your insurance number available so your benefits can be determined in advance. You may also want to check with the Human Resource person at your company to check out your benefits.

Blue Cross Blue Shield
BCBS is a vision care plan that we accept. If you have this plan again it would be wise for you to check with the insurance company or your office to see what your benefits are towards eyecare and materials before you come for your appointment. When scheduling make sure that you let us know that you have vision care benefits so we can verify them in advance.

Other Vision Insurance Plans
There are other vision insurance plans that our office does not accept but new plans come out frequently and we are willing to see if they are good for our patients and our office. If you are on a plan that we do not take, it does not mean that we cannot provide services. Many times our patients may have a vision care plan and want to use our services. They check with their benefits department if it is a concern and find out what they may be reimbursed if they use our services and/or get glasses from us.

Medical Insurance Plans
We are listed under Medical Optometrists who can treat eye disease depending on the diagnosis for about 5 major medical plans. Check your plan and see if Dr. Warren Johnson is on the list. If there is a medical reason for your visit then we will accept assignment of the charges and file with your major medical carrier. Again we would need to know this in advance to be aware of your benefits. There may be additions or deletions to this list so please check with your benefits department:

• United Health Care

* Medicare is a plan that we can file for you and take assignment on after cataract surgery. It does not pay for glasses or towards glasses except after cataract surgery.
